  I cannot explain the depth of my disappointment in Edward-Elmhurst office and billing department. I was a lifelong patient there. Last year my medication needed prior authorization to be sent to a mail in pharmacy, the nurse sent me emails rather than calling me arguing about doing so. I called the office and asked them to please do the prior authorization. They never did. Two weeks later another nurse called to fight with me about it more even though I had already filled the Rx so it was too late anyway. I also had my appointments cancelled on me, one staff member would say a time was available and another would say it was not on many occasions. The straw that broke the camels back for me though, was this year. I called and scheduled my regular annual visit with Jennifer Chludzinski, PA. Unbeknownst to me the scheduler made the appointment a follow up rather than an annual visit even though I had explained to her on the phone that was what I wanted and that I could come 10 months rather than 12 months, my insurance would cover it. Jennifer wrote the note as a follow up for a diagnosis I do not have and I was sent a bill. When I tried on Numerous occasions to call the office and billing department for help I was treated terribly and always promised a call back which never would happen. I work in the medical field and if I had ever spoke to a patient the way their staff and office manager spoke to me I would be fired. I followed up with a letter directly to Jennifer, the billing department and to BCBS and Jennifer, the office nor the billing department ever even acknowledged it. Upon requesting my records I have found errors which are very concerning and show dishonest record keeping. I will never go back to this office now that I am aware they send out fraudulent claims and put false information in office notes.
